Your birthday tells you how many years have passed. It doesn’t tell you what those years have done to your body. A new generation of diagnostic testing claims to provide answers, going beyond routine blood work to estimate how your body is aging internally, where it may be under stress and what that could mean for your health down the road. The options run from the clinically grounded to the largely speculative. We talked to three Southwest Florida physicians about what these tests can realistically offer and where the limits are.
What's Being Measured
Biological age testing is among the fastest-growing categories in diagnostics. Unlike the wellness scores generated by wearable devices such as Apple Watches and Whoop trackers, these clinical assessments look at the cellular level to estimate whether your body is aging more quickly or slowly than the calendar suggests.
Leveraging advanced computational models and large datasets, the tests analyze an individual’s biological profile to identify patterns associated with aging, disease and mortality. A 42-year-old whose biological age comes back at 50, for example, may be flagged as vulnerable to age-related disease.
There is no standard measurement for biological age. Send your sample to three labs, and you may get three different ages. Some tests look at DNA, others at immune system antibodies, blood proteins, or a combination of chemical and physical markers to estimate the cumulative wear and tear on cells. The oldest and most studied are epigenetic tests, which measure DNA methylation—changes in chemical tags that influence gene expression. Newer tests can get even more specific, depending on which blood proteins they measure.
Unlike chronological age, biological age is dynamic, shifting in response to lifestyle, environment and treatment. A patient who changes their habits can retest in six to 12 months to see whether the markers have moved. But the scores are only statistical probabilities, not diagnoses.
Beyond cellular testing, standard lab work is also growing more detailed and accessible. Direct-to-consumer company Function Health offers advanced blood panels for a few hundred dollars, with testing conducted by Quest Diagnostics or Labcorp. The membership-based platform screens for more than 100 biomarkers (standard annual exams run about 10 to 30), meant to identify subclinical imbalances and potential root causes of dysfunction in the body.
Where biological age tests aggregate findings into predictive health scores, these panels provide raw data. Function Health offers metabolic, cardiovascular and hormonal health data that you can discuss with your doctor, according to Naples board-certified internist Dr. Corey Howard.
Dr. Jim Liu, of Elevated Concierge & Longevity Medicine in Bonita Springs, recommends the Genova Diagnostics’ NutrEval 3000, a blood and urine test that screens for specific nutrient deficiencies, heavy metals and environmental exposures. It can be particularly useful, he notes, at a time when people are taking all manner of supplements that are not proven to be medically sound unless there’s a deficiency.
Physicians also look at VO2 max, measured with a treadmill or stationary bike stress test, to determine how much oxygen your heart, lungs and muscles can deliver and use. It remains one of the strongest long-term predictors of health span, the number of years a person remains active and free of chronic disease. Studies show that a low score may be a stronger predictor of death than smoking, diabetes and hypertension.

How Physicians Use the Results
For doctors, the goal isn’t to help patients live longer at any cost but to improve health span. “Most of my patients would rather have a shorter life with a longer span of full functionality than a longer life diminished by frailty,” Dr. Liu says.
Often, the patients seeking advanced preventive profiling, says Dr. Jonathan Marsh, of Naples Concierge Healthcare, are mostly people in their 50s and 60s who are already healthy. They know they need to move often, eat well and prioritize sleep. But they want a more personalized picture of what they need, how their body is aging and whether something may be slipping by— especially if they watched an otherwise healthy parent develop Alzheimer’s, or had a health scare traditional testing didn’t catch. “They worked so hard. Sometimes they’ve done some damage along the way—client dinners, travel, stress—and now they want to start making up for it,” he says.
Ever-growing bio age tests measure DNA biomarkers for Alzheimer’s disease or cancer risk, hormone levels, vitamin and macronutrient deficiencies, and bone health, among other health indicators. If someone’s standard labs look clean but a test flags their liver as seven years older than expected, a physician can pursue further imaging.

Consumer biological age tests and advanced screenings aim to identify internal patterns in aging, disease risk and overall health before they show up in symptoms or routine lab work. The data is only as useful as the physician interpreting it—and some of what’s being marketed is still well ahead of the science.
These tests are a natural fit for a concierge or functional medicine practice, where appointments are longer and follow-through is more individualized than a standard annual physical allows. Physicians treat the results as a baseline—a starting point for a personalized plan and a way to track the impact of lifestyle changes or treatments. They often combine various assessments. At his concierge clinic, Dr. Howard uses hormonal and metabolic panels, along with a few scientifically validated biological age tests. He runs the epigenetic TruAge and DunedinPACE assessments and GlycanAge, which examines immune antibodies to measure chronic inflammation. Together, they give him a more complete picture of a patient’s health and how to best treat them.

What the Numbers Can't Tell You
Many of these tests are now available directly to consumers. “Some people would probably be surprised that it can be done for what they might consider a reasonable amount of money,” Dr. Marsh says. TruAge, the most common direct-to-order epigenetic test, runs around $500 and is done at home with a finger prick. Function Health costs $400 for an annual membership. The full Genova3000 assessment, recommended by Dr. Liu, can only be ordered by a licensed, in-state physician, and can be billed to your insurance company.
Depending on the test, patients may receive organ-by-organ aging estimates, inflammation and metabolic scores, disease-risk projections, pace-of-aging measurements and personalized recommendations spanning diet, supplements, exercise and sleep. For some patients, that level of detail is clarifying, giving them a concrete reason to address the thing they’ve been ignoring, whether that’s alcohol consumption, sleep or stress. For many others, it’s too much to process, raising more questions than answers or creating unnecessary worry.
However, physicians advise against interpreting results from any blood panel without professional input. “It’s so complex that I wouldn’t advise anybody to tackle it without a doctor who has regular medical training to help guide them,” Dr. Marsh says. The apparent precision of the results can also exceed the certainty of the science behind them. Researchers are still determining how clinically actionable certain markers are. A test could also present a false flag while missing an acute problem. “You’re going to have to mentally be OK with something flagged that doesn’t actually mean anything,” Dr. Marsh says.
Then there is the question of what you might not be prepared to hear. Dr. Liu’s first question for any patient is why they are requesting the tests. Someone worried about Alzheimer’s needs a different conversation than someone focused on metabolic health. “We can test for 300 different things, but there’s a balance,” he says. “You look for the things that are most important.” He also works to manage expectations. A patient who is 70 and testing at 62 may feel they’ve beaten the odds, but a single health event can erase that gap.
The gap between longevity marketing and hard clinical reality is a common theme in preventive care. Some testing companies package results with a menu of supplements and therapies that require equal scrutiny. The longevity industry promotes peptides, NAD infusions, exosomes and IV therapies, but evidence supporting them for anti-aging purposes is limited. “There are 300,000 peptides in the human body. We hardly know any of them,” Dr. Howard says. Other treatments, like red light therapy and platelet-rich plasma, fall into a grayer area. Evidence on their effectiveness is weak at best, but they’re generally not harmful, according to Dr. Marsh. “They’re the icing on the cake, but they’re not the cake,” he says.
For all the sophistication of modern diagnostic testing, the body remains more complicated than any dashboard can say. The goal is not to achieve certainty or perfect stats, but to understand a little more about where you stand today—and to use that information wisely.
